Healthpeak Properties, Inc., the owner of Westpark Plaza, and Lincoln Harris CSG, agent for owner, require that we have your current insurance certificate on file before the commencement of occupancy or any construction project. The Certificate of Insurance should meet the following requirements:

1. Worker’s Compensation and Employer’s Liability Insurance, which shall fully comply with the statutory requirements of all, state laws as well as federal laws, which may be applicable. Employer’s Liability limit shall be in accordance with applicable law per accident for Bodily Injury and employee/aggregate for disease.
   
   
2. Commercial General Liability insurance with a minimum combined single limit of liability of $1,000,000 per occurrence and $2,000,000 aggregate for injury and /or death and/or property damage.
   
   
3. Business Automobile Liability Insurance covering all owned, hired, and non-owned vehicles and equipment used by the Contractor with a minimum combined single limit of liability of $1,000,000 for injury and/or death and/or property damage.
